As you get older, the loss of skin elasticity is also visible in your eyelids, with the appearance of excess skin. Also, in the lower eyelids, fat herniates and so-called “under-eye bags ” appear, giving your face an aged look.Blepharoplasty is a surgical eyelid correction procedure for people of both sexes, generally over 40, although in the case of hereditary changes, it can be performed even earlier. Eyelid correction will make you look more rested, younger, fresher.

Blepharoplasty (sagging eyelid correction surgery) is surgery to remove excess skin, fat and muscle from the upper and/or lower eyelids in order to restore the appearance of the eyes. I, Dr Bortolini, can correct both the upper eyelids (excess skin) and the lower eyelids (bags under the eyes) in the same operation or separately. Your health is assessed (possible diseases, allergies, usual medications). Afterwards we recommend blood tests and sometimes an ophthalmological consultation.

HOW IS BLEPHAROPLASTY SURGERY PERFORMED?

Blepharoplasty can be performed under local anesthesia only, local anesthesia combined with intravenous sedation, or even under general anesthesia. During the consultation we will determine together which is the best option for you.

The blepharoplasty operation lasts between 30 minutes and 2 hours and depends on whether the lower eyelids, upper eyelids or both eyelids are operated on.

The incision in the upper eyelid is hidden in the eyelid crease, and is very inconspicuous or not visible at all, depending on the scarring of each individual person. Excess fat, skin and possibly muscle from the upper eyelid is then removed.

In the case of the lower eyelid, the incision is on the outside, 1 – 2 mm below the lashes (free edge of the eyelid). If only excess fat needs to be removed, a transconjunctival incision can be used (on the inside of the eyelid, the incision is invisible).

This removes excess fat, skin and possibly muscle. The sutures are made with very fine special sutures which are then removed a few days postoperatively to create a minimal scar.

AFTER BLEPHAROPLASTY SURGERY

  • After blepharoplasty surgery in most cases there is no need for hospitalization if only local anesthesia was used.
  • Postoperative pain may be absent or even minimal. Bruising (ecchymosis) and swelling (oedema) in the operated areas disappear within two weeks.
  • You can wear sunglasses from the second day after surgery, but wearing contact lenses should be avoided 10-14 days postoperatively.
  • In the first days after the operation you should follow an analgesic and antibiotic treatment for 5-7 days.
  • Physical exertion and sports should be avoided for at least 2 weeks, and then gradually resumed.
  • Check-ups will be at first every 2-3 days and then every 3, 6, 12 months.
  • In general it takes six months to be able to assess the outcome of your surgery. The removal of the fat “bags” around the eyes is practically definitive, with no risk of recurrence.
  • But the skin continues to age and sometimes the excess in the upper eyelids can slowly reappear (rarely a new operation may be necessary more than 10 years after the first operation).

What are the risks of blepharoplasty?

Common Risks

  • Hematoma: May require intervention for drainage.
  • Infection: Controllable with antibiotics.
  • Reactions to anaesthesia: Adverse reactions may occur.
  • Scarring: Scarring may be more prominent in some patients.

Specific Risks

  • Altered sensitivity: Temporary changes in skin sensitivity.
  • Asymmetry: Sometimes additional correction is required.
  • Vision problems: Double vision or difficulty closing the eyelids are usually temporary.
  • Ectropion: Rarely, the eyelid edges may turn outwards.

Long-Term Risks

  • Changes in eyelid position: further surgery may be required.
  • Chronic ocular sequelae: Dry eyes may require long-term treatment.
