Men in general end up having problems with the appearance of the nose after certain altercations, from sports or other traumas that change the shape of their nose and also that facial harmony. This is the moment when even the nasal functions are often affected and surgery is necessary to solve breathing problems and to restore the shape of the nose so that it is no longer an aesthetic problem.
In recent years, men have also started to resort to cosmetic surgery, especially rhinoplasty to remedy certain birth or post-traumatic deficiencies to gain more self-confidence, which will be felt both personally and in social relationships. Most of the time, the shape and size of the nose is a sensitive subject for many teenagers and young people. The mischievous nicknames, the malicious comments, the jokes made by peers and other people in relation to it can lead to an unharmonious development of the adolescent’s personality and therefore, if the state of health allows, it is ideal to remove aesthetic defects at an early age. Rhinoplasty surgery can be performed after the age of 16-18. A large nose, with a defective position, an unsightly hump, is an important factor in the imbalance of the harmony of facial features and therefore, nose reshaping is the ideal choice.
A nose that is too long or too wide can be corrected, a hump can be reduced, a voluminous or “droopy” tip can be reduced, a nose that is too large overall can be reduced, breathing disorders can be corrected, etc. The aim of this operation is to achieve a natural looking nose, in harmony with the other facial structures and without breathing problems. Most surgeons rightly consider rhinoplasty as one of the most complex and sophisticated surgical procedures. Rhinoplasty or nose reshaping surgery improves the appearance and proportions of the nose as well as the harmony of facial features and self-confidence. Nose surgery can also correct breathing problems caused by structural defects of the nose. A nasal surgery to improve obstructed airways requires a careful evaluation of the structures of the nose. Correction of a deviated septum and nasal turbinate hypertrophy are the most common causes of breathing problems

Your health will be assessed (possible diseases, allergies, usual medications). Afterwards, blood tests and X-rays of the nasal pyramid or CT (computer tomography) are recommended.
Before the operation it is recommended to avoid the use of anti-inflammatory drugs or aspirin for at least 14 days prior to the operation (they promote bleeding).
It is also recommended to stop smoking at least 1 month before the operation or at least to reduce considerably the number of cigarettes smoked per day (it hinders the healing process and complications may occur).
On the day of the operation you will not eat or drink anything for at least 6 hours before the operation.
The operation lasts 2-4 hours depending on the complexity of the changes made requires general anesthesia and 1-day hospitalization.
I practice only open rhinoplasty because it is the most complete and complex method by which all the necessary aesthetic and functional (respiratory) changes can be made.
Only one incision is made on the outside (3 mm), at the columella level (the area that separates the nostrils) where the rest of the incisions are not visible and are located inside the nostrils. Rarely (if the nostrils need to be narrowed) it may be necessary to make an incision in the groove separating the nasal ala from the rest of the face.
Through these incisions, the soft tissues covering the nose are gently lifted, providing access for reshaping the structure of the nose. Nose surgery can reduce or, conversely, augment nasal structures by using cartilage grafts harvested from other areas of the body.
Most commonly, pieces of cartilage from the nasal septum, from the middle portion of the nose, are used. In some cases, a piece of cartilage from the ear and, rarely, rib cartilage may be used.
If the septum is deviated, it will be straightened and the projections inside the nose are reduced to improve breathing. Once the supporting structure of the nose is shaped into the desired shape, the skin and tissues are reshaped and the incisions are closed.
In most cases the nostrils are then filled with tables that will be held for 24-48 hours. On the outside, a bandage is made of adhesive strips over which a plastic nasal splint is then applied.
International experts in the field of plastic surgery recommend that following rhinoplasty surgery in men, the nose should not be hypercorrected and an effeminate nose (a common mistake) should be produced. The male nose should not be “turned up”, nor should it be “hollow” or too small, it should be straight, beautiful but “strong” and express masculinity.
Postoperative pain is usually minimal. Bruising (ecchymosis) and swelling (oedema) of the operated areas disappear within two weeks.
In the first days after surgery, analgesic and antibiotic treatment must be followed for 5-7 days. The splint is removed 1-2 days postoperatively and the splint is removed after 10 days.
Although the initial oedema recedes in a few weeks, it takes about a year before the final shape of the nose is definitive. During this time you will notice gradual changes in the appearance of the nose as it evolves towards a permanent result. In the first year after surgery, swelling may appear and disappear, being more pronounced in the morning.
In the first 2-3 weeks avoid blowing your nose. The same goes for wearing glasses during the first 2 months (the period of time when contact lenses are recommended).
Generally in 10-12 days you can resume daily activities, and strenuous physical activities can only be resumed after at least 2 months.
After the operation, internal tables or tubes can be placed inside the nose and a splint and dressing will be present on the outside to support and protect the new structures during the initial healing phase.
Although the results of rhinoplasty are generally permanent, unfortunately the cartilage can continue to reshape and displace the tissue, which can alter the results of the operation over time.
You should come for a check-up at the beginning every 2-3 days to 2 weeks when the sutures are removed and then at 3,6,12 months.
Postoperatively there will be bruising and swelling in the operated area. Discomfort and pain will be controlled with the help of analgesic medication for 5 days.
A dressing is worn for a few weeks to reduce swelling and adjust the skin to the new contour of the chest. Although the results will be visible almost immediately, the swelling will prevent you from assessing the final results. These will be visible in a few weeks after the operation.
Most patients will be able to resume their normal activities in 2-3 days.

Massage of the liposuctioned areas can be started as soon as the bruising and swelling have disappeared.
