Deep Plane Extended Facelift

Without a doubt, the Deep Plane Extended Facelift is the most powerful, yet completely natural way to restore true youth to an aging face. While skin-only facelifts have been outdated for years, most modern facelift surgeons still perform SMAS or even deep plane procedures that rely on tensing or pulling the muscular layer of the face to achieve the facelift. The muscle bears all the tension and the skin is passively closed. This is touted as state of the art.

True rejuvenation, however, must be completely tension-free – even on the muscle layer – to prevent strained-looking results and early failures.

Achieving a tension-free lift of the muscle layer requires complete, passive release and repositioning of the deep structures of the face, which few surgeons can do safely and reliably.

It is this completely passive repositioning of fallen facial structures back to their original positions that sets Dr. Bortolini’s results apart and attracts patients from across the country and around the world.

The Deep Plane Extended Facelift is, in almost all cases, performed in conjunction with a Deep Necklift, and the listed price includes both procedures together.

With advancing age the facial contour loses its firmness due to the aging process of the skin. If you too have noticed looking in the mirror that your eyes have lost their brightness over time because your eyebrows are drooping, your cheekbones are sagging, your nasolabial folds (the wrinkles between your nose and mouth) are appearing, and your jawline has become more saggy, then you can benefit from a facelift! All this can be corrected by facelift surgery(facial rejuvenation effect) thus lifting and rejuvenating your entire face! There are several types of facelift techniques depending on the degree of tissue laxity and the surgery ranges from minimally invasive to extensive, sophisticated procedures:

  • Minilifting
  • MACS facelift (facelift + suspension wires)
  • Lifting with plication or sectioning of the SMAS.
  • Total facelift
  • Face and neck lift

The procedure is recommended if you have :

  • Sagging skin, central and lower facial area.
  • Tired facial appearance and a sad face.
  • Cheeks with a sagging appearance.
  • Fat deposits in the neck (neckline) with a “double chin” or “turkey neck” appearance.

Facelift (face rejuvenation effect) can be combined for a superior result with upper and lower blepharoplasty and botulinum toxin to remove wrinkles in the upper face (forehead area, between the eyebrows or at the corner of the eye). To find out which technique suits you, schedule a consultation and I, Dr. Bortolini, specialist in plastic surgery, will answer all your questions!

BEFORE FACELIFT SURGERY

During the consultation we will discuss everything about the facelift surgery, its risks and limitations and I will answer all your questions. We will take measurements and together we will choose the best option for you.

Your health is evaluated (possible diseases, allergies, usual medications). Afterwards, an abdominal ultrasound and blood tests are recommended.

Before facelift surgery (face rejuvenation effect) it is recommended to avoid the use of anti-inflammatory or aspirin-type drugs or anticoagulants (they promote bleeding) for at least 14 days before the surgery.

Contraceptives should be stopped 4-6 weeks before surgery because they increase the risk of thromboembolic complications (blood clots).

Facelift

It is also recommended to stop smoking at least 1 month before or at least to reduce considerably the number of cigarettes smoked per day (it hinders the healing process and complications may occur).

The facelift operation should not be performed during the menstrual cycle.

On the day of the operation you will not eat or drink anything for at least 6 hours before the operation.

FACELIFT SURGERY

Facelift

A facelift removes excess skin, strengthens subcutaneous tissues and muscles and repositions the skin on the face and neck.

Incisions vary depending on the type of facelift chosen and the extent of the procedure.

In minimally invasive variants, the incision is only in the temples and in front of the ears up to the ear lobe.

In the total facelift the incisions extend behind the ears and at the hairline (behind the ears).

If a neck lift is combined with a cervical lift (in the neck area) to treat “double chin” or “turkey neck” there is another incision that will be hidden under the chin.

After the incisions are made, it will separate the skin from the deeper tissues. This frees the skin to be redraped and tightened, achieving a smooth appearance.

The subcutaneous tissue will be suspended and plicated (SMAS – plication) with the help of wires or the excess will be removed and then sutured (SMASectomy) and the excess skin and fat will be removed.

If a neck lift is also associated, the fat in this area can be liposuctioned and the muscles plicated to restore firmness to the cervical (neck) area.

Finally, after the skin has been tightened and the excess removed, the incisions are sutured with a special type of intradermal suture that will leave minimal (almost imperceptible) marks.

If necessary, drainage tubes will be installed and the operated area will be dressed and an elastic corset applied.

AFTER FACELIFT SURGERY

Facelift

Face rejuvenation surgery is done under general anesthesia and hospitalization is 1 day.

Postoperative pain is usually of low intensity. Bruising and swelling (oedema) although they can be quite significant at first, disappear within two weeks.

You will wear an “elastic mask” on your face and neck for 2 weeks.

If drainage tubes are needed they will be kept in 24-48 hours postoperatively.

It is important to keep your head elevated and not bent forward;

In the first days after the operation you should be given analgesic and antibiotic treatment for 5-7 days.

Intense physical exertion should be avoided for 1 month.

You should come for a check-up every 2-3 days to 2 weeks and then every 3,6,12 months.
