Abdominoplasty is the surgery that removes excess skin and fat from the abdomen and strengthens the abdominal muscles. Pre-existing abdominal scars (e.g. caesarean section) can also be removed during the operation. Abdominoplasty is a procedure that can be combined with liposuction, and many patients are recommended both procedures.
Abdominoplasty (like liposuction) is not weight loss surgery and is not a substitute for a healthy diet or exercise, but in combination with a healthy lifestyle it can give you the results you want.
It is best to lose weight initially (through diet, exercise or bariatric surgery), and then once you have achieved a stable weight over time and have excess skin and a little fat, to have abdominoplasty surgery to shape your new body. If you have lost a lot of weight, you may even need a more complex post-bariatric surgery.

During the consultation we will discuss everything involved in the surgery and I will answer any questions you may have. We will measure, discuss and choose the best option for you. Afterwards, an abdominal ultrasound and blood tests are recommended.
Your health will be assessed (possible illnesses, allergies, usual medications). An abdominal ultrasound and blood tests are then recommended.
Before the operation it is recommended to avoid the use of anti-inflammatory drugs or aspirin (they promote bleeding) for at least 14 days before the operation.
Contraceptives should be stopped 4-6 weeks before surgery because they increase the risk of thromboembolic (blood clotting) complications.
Excessive exposure to the sun or the sun should be avoided before surgery.
It is also recommended to stop smoking at least 1 month before the operation or at least to considerably reduce the number of cigarettes smoked per day (it hinders the healing process and complications may occur).
The operation is NOT performed during the menstrual cycle.
On the day of the operation you must not consume food and liquids for at least 6 hours before the operation.
It is recommended to avoid gel nails or nail polish because they interfere with the anaesthesia machines and body creams in the morning.
The duration of the surgery is, on average, 3-4 hours.
Abdominoplasty is usually performed under general anaesthesia. In some cases (miniabdominoplasty, lipoabdominoplasty), spinal anaesthesia may also be performed.
The incision is horizontal, across the entire width of the abdomen, located in the lower region of the abdomen, at the level of the pubic hair. The abdominal skin and fat are detached from the muscular layer underneath. The muscle is then plicated with special wires (to narrow the waist and correct post-pregnancy muscle defects), any areas of weakness or hernia are repaired.
Excess skin and fat (usually represented by the area between the navel and pubic area) is removed, with the navel preserved. The integument above the navel is then pulled down and sutured to the lower integument. The umbilicus is externalised in a new position through a small incision made in the skin.
Such surgery always leaves a horizontal scar in the lower part of the abdomen, of varying length, which is usually hidden in the bikini line and a circular scar around the new navel.
In limited cases where only a small correction is needed, miniabdominoplasty may be performed. The horizontal scar is shorter, there is no scar around the navel and recovery is easier.
In some cases abdominoplasty can also be combined with liposuction surgery to reduce fat deposits in the upper abdomen and flanks.
At the end of the operation, an elastic corset is applied to the treated areas.
Hospitalisation for abdominoplasty surgery is approximately 1 – 3 days (depending on the extent of the operation, type of anaesthesia, general condition of the patient).
I, Dr. Bortolini, recommend that during the first days of recovery you rest well, hydrate, meals should be light, small and regular.
Postoperative pain is usually of medium intensity. However, it should be borne in mind that the operation is quite extensive and recovery will sometimes be long. Bruising (ecchymosis) and swelling (oedema) of the operated areas disappear within two weeks.
Although it is a larger operation, the spectacular post-operative results are worth all the effort you put in!
In the first days after the operation, you should follow an analgesic and antibiotic treatment for 5-7 days.
The drainage tubes are removed after a day or two.
The sutures can be removed (if absorbable sutures are used), or removed 14 days postoperatively.
It is necessary to interrupt activity for about 2 weeks postoperatively.
Physical exertion should be avoided completely for the first 2 months and then resumed progressively.
Postoperative scars will be slightly red for the first few months postoperatively, after which they tend to fade with time (up to 1-2 years postoperatively). Prolonged sun exposure or sun exposure should be avoided for the first 9-12 months postoperatively.
The elastic corset should be worn for a minimum of 4-6 weeks to encourage the skin to settle.
Post-operative check-ups will be at first every 2-3 days to 2 weeks and then at 3,6,12 months.
Although the results of abdominoplasty are permanent, there are cases where they may be influenced by large weight fluctuations or pregnancy. For this reason, if you want to lose substantial weight or if you are planning a pregnancy, it would be better to postpone this type of surgery until you achieve a stable weight loss over time.
