All about breast implant placement

If you’ve been asking about breast implants or if you have friends who have implants you’ve probably heard that it takes a certain amount of time for them to “set”. What exactly does this mean? Most of the time (almost all of the time) I, Dr Bortolini Alin, a plastic surgeon, place breast implants in the subpectoral position. Actually more accurately and correctly they are positioned partially subpectoral or dual plane/Tebbets (after the American surgeon who described the technique) When breast implants are placed under the pectoralis major muscle they are initially flattened by the muscle and sit high on the chest, swollen in the upper pole due to intense inflammation of the pectoral muscle. They are also hard and stiff – immobile (often described by patients as two stones 😊), all these things are temporary and they will become softer and more supple from month to month.
With the passage of time the implant stretches the pectoral muscle and the breast tissue (breast) and the implant “settles” and “sinks” more into its pocket and the breast rounds out and becomes fuller and contours better in the lower pole while the upper pole “deflates” more and the areola which at first may look down now rises more. This process depends from person to person but most of the time it happens about 3 months after the surgery. Another thing to consider is that immediately after the surgery the breast area (between the breasts) is very swollen and the implants may look “smaller” and less contoured. Once the swelling and “deepening” of this area is gone, they become much better contoured and the breast looks fuller and more contoured in the cleavage. To put it in a more plastic way, seen from above they look like two small valleys with a small valley between them, but with the deflation of this area, the “valley” widens and makes them stand out much better by contrast. Below are some examples, with my personal cases, of the evolution of breast implants over time to better understand why you need to be patient when it comes to evaluating the results.
